Activity work plan reports
The AWP report details a Reconnect service provider’s progress against their approved AWP.
This demonstrates how Reconnect service providers have met the obligations under their Reconnect grant agreement.
It also informs conversations about service delivery between service providers, the Community Grants Hub and Treasury.
The service provider’s grant agreement specifies when the AWP report is due.
An AWP report uses the same template as the AWP.
Submitting activity work plans and reports
Reconnect service providers should complete and submit their AWP and AWP Report to the Community Grants Hub. They should follow the reporting intervals outlined in their grant agreements.

Data Exchange requirements for Reconnect service providers
The Data Exchange (DEX) partnership approach is a two-way sharing of information.
It helps service providers:
- find smarter and more efficient ways of improving service delivery
- understand the overall outcomes achieved for individuals, families and their communities.
All Reconnect service providers must follow the partnership approach under DEX.
DEX provides practical, program-specific information for managers and front-line staff for:
- what data they’re expected to collect
- when and how they should collect it.
It also helps integrate Standard Client/Community Outcome Reporting (SCORE) data collection into service and administrative practices.
Critical incident reporting
All Reconnect service providers must notify their Community Grants Hub funding arrangement manager of critical incidents either:
- within 48 hours of an incident happening
- within 48 hours of becoming aware of a critical incident happening.
To notify your funding arrangement manager:
- follow the critical incident guidelines
- complete the form
- email it to the Funding Arrangement Manager listed in your grant agreement.
